视网膜母细胞瘤化疗的眼科药理学

　　年9月8日，美国眼科交流学会官方期刊《视网膜》在线发表医院、阿根廷国家科学技术研究委员会、荷兰阿姆斯特丹自由大学医学中心、美国纽约纪念斯隆凯特林癌症中心的综述，通过文献检索对用于眼内视网膜母细胞瘤的三个最重要化疗药物（卡铂、马法兰、托泊替康）临床前和临床药代动力学研究进行了系统回顾以优化治疗。

　　结果共有21项研究评定了通过静脉、眼周、眼动脉和玻璃体内注射卡铂、托泊替康（拓扑替康）、马法兰（美法仑）的临床前和临床药代动力学。某些临床前研究在临床转化前进行。除了眼周注射托泊替康报告了令人鼓舞的临床前数据，其他与临床应用无相关性。此外，虽然无有利的临床前信息，但是根据马法兰用于眼动脉化学外科的病例，某些给药途径在临床上是有效的。除了托泊替康，仍然缺乏马法兰和卡铂药代动力学的完整知识。

METHODS:SystemicreviewofpharmacokineticstudiesidentifiedbyaliteraturesearchatPubmedusingthekeywordscarboplatin,melphalan,topotecan,intravitreal,ophthalmicarterychemosurgery,pharmacokinetics,andretinoblastoma.

RESULTS:Atotalof21studieswerereviewedforassessingthepreclinicalandclinicalpharmacokineticsofcarboplatin,topotecan,andmelphalandeliveredbyintravenous,periocular,ophthalmicartery,andintravitrealroutes.Somepreclinicalstudiesweredonebeforetranslationtotheclinics.Others,despiteencouragingpreclinicaldataasreportedforperioculartopotecandidnotcorrelatewithclinicaluse.Inaddition,aswasthecaseformelphalanafterophthalmicarterychemosurgeryanddespitenonfavorablepreclinicalinformation,someroutesofdrugdeliveryareclinicallyeffective.Besidestopotecan,
